Funded by the Ontario Ministry of Health and in partnership with the Centre for Addiction and Mental Health’s (CAMH) Smoking Treatment for Ontario Patients (STOP) Program, PAARC’s Smoking Cessation program provides counselling support and up to 26 weeks of Nicotine Replacement Therapy (NRT) during a one year enrollment period at no cost to any individuals who wish to reduce and/or quit their nicotine and tobacco use. Please contact PAARC intake at (905) 629-1007 ext. 2222 for further information regarding this program and eligibility criteria. An individual may self-refer by contacting PAARC intake or a referral from a health provider or community agency can be sent on the individual’s behalf. A physician referral form* to confirm that the individual is medically stable and a good fit for NRT must be completed prior to enrollment in the program – this form is provided by PAARC staff and can be faxed to a physician with consent on your behalf.

*Please note that a fee may be charged for this form and can vary from physician to physician.
